Tadacip [tadalafil], a product of Cipla is a selective inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5). Erectile dysfunction

Erection of the penis during coitus or otherwise is a result of increase in the blood flow of the arteries of the penis and simultaneous relaxation of the smooth muscle of corpora cavernosum. This whole process is due to the release of nitric oxide (NO) from nerve terminals and endothelial cells, which stimulates the synthesis of cGMP in smooth muscle cells. Cyclic GMP in turn, relaxes smooth muscle and resulting in an increase in the blood flow to the corpus cavernosum.
In erectile dysfunction, enough blood does not flow in the arteries of the penis to cause a sufficient erection.
Causes of erectile dysfunction:

psychological problems, including anxiety
vascular insufficiency [atheroma]
neuropathic [diabetes mellitus, alcohol excess, multiple sclerosis]

DOSAGE AND ADMINISTRATION
The recommended starting dose of TADACIP is 10 mg, taken prior to anticipated sexual activity. The dose may be increased to 20 mg or decreased to 5 mg, based on individual efficacy and tolerability. The maximum recommended dosing frequency is once per day in most patients. The dose should not exceed 20 mg in any circumstances. TADACIP was shown to improve erectile function compared to placebo up to 36 hours following dosing. Therefore, when advising patients on optimal use of TADACIP, this should be taken into consideration. TADACIP may be taken without regards to food, since food does not show any effect on the absorption of the drug. It is important to update the patient on the fact that TADACIP alone does not cause erection of the penis and that self stimulation or with the help of the partner is required.
TADACIP [tadalafil] has a pharmacologic distinction of having a longer half-life of 17.50 hours, meaning it maintains its potency for at least 17.50 hours. — compared to other drugs of the same class like Viagra (4.0–5.0 hours) and Levitra (4.0–5.0 hours) — resulting in longer duration of action. Due to this unique pharmacological property, TADACIP has earned the sobriquet of “The Weekend Pill”
It is also in the clinical trials for treatment of pulmonary arterial hypertension.
SIDE EFFECTS
Serious side effects:
visual disturbances;
hearing loss, ringing of ears [tinnitus];
chest pain;
irregular heartbeat [arrhythmia];
swelling of hands, feet, ankles;
difficulty in breathing;
seizure (convulsions);
feeling light-headed, fainting; or
Penis erection that is painful or lasts 4 hours or longer.
Less serious side effects may include:
warmth or redness in face, neck, or chest;
stuffy nose, sore throat;
headache;
diarrhea; or
Muscle pain, back pain.
CONTRAINDICATIONS
Nitrates [tadalafil can potentiate the hypotensive effect of nitrates]
alpha blockers [a significant augmentation of the blood-pressure-lowering effect of doxazosin]
hypersensitivity
OVER DOSAGE
Symptoms of overdose may include:
headache
indigestion or heartburn
flushing
pain in the back, muscles, or any limb
stuffy or runny nose
erection that lasts longer than 4 hours
